What to have ready

• Participant contact details and preferred contact method: email, phone or text. Add a representative’s details if applicable.
• NDIS funding and employment goals, previous experience with a Disability Employment Services provider, recent functional assessment and any behaviour support plan.
• Education, qualifications and certificates, current or previous employment, a CV if available and your reasons for wanting to work.
• The form asks separately for permission to contact your primary support person, DES provider and current or previous employers.
• Documents can be uploaded on the original form: up to five files, with a listed maximum file size of 5 MB. Review the live upload instructions before sharing.
