Before you begin
- You may enquire for yourself or another person. The form asks for your relationship to that person.
- Have your name, email address and phone number ready, with a brief explanation of the support needed.
- The form asks about NDIS approval or assessment status, support coordination and whether the plan is self-managed. Options include waiting for an outcome and not being eligible yet.
What happens next
The source enquiry pages state that a team member will be in touch within two business days to discuss next steps. If you need to check the progress of an enquiry, call 1300 133 660.
