Participant and contact details

• The form asks for the participant’s full name, date of birth, gender, pronouns, address, email and phone number.
• Provide the primary contact’s name, phone, email and relationship to the participant. A different home address can be added where relevant.

Support and access needs

• The original form asks about a medical diagnosis, support requirements, NDIS plan and mobility issues.
• It also asks where you heard about A Weekend Away. Submitting interest does not confirm a place or booking.
