An approved Support at Home package can bring together services to help you live independently. JewishCare works with you to tailor support to your care needs and goals.
Government-funded support depends on an assessment and approval. My Aged Care can explain the application process and arrange an assessment. JewishCare can help you understand the pathway.
The funding and any contributions depend on your circumstances and approved services. Use the current Support at Home information and price guide, and discuss costs before arranging care. Historic package-level tables are not a guide to current entitlements.
A Care Partner can review your care plan when your circumstances change and help seek a new assessment if required. Services remain subject to approved funding and programme requirements.
Support with showering, bathing, dressing, grooming, toileting and continence, getting in and out of bed, medication reminders and everyday routines. Your care plan is shaped around your needs.
Cleaning, laundry, light household tasks, meal preparation, shopping and garden maintenance can help you remain comfortable at home. Support with delivered meals excludes the cost of food.
Care Partners listen to you and your family to develop a person-centred plan. Services can be reviewed as needs change, within available funding and programme rules. Our team supports cultural needs; many staff speak Russian, Hebrew, Polish or Hungarian as well as English.
Contact JewishCare to discuss your needs. If you are enquiring on someone else’s behalf, make sure you have their permission. The current aged-care enquiry form accepts enquiries for yourself or another person and states a response within 2 business days.
Talk with your Care Partner. Your care plan can be reviewed, services adjusted within programme rules and available funds, and a new assessment arranged where needed.
The team gathers information about your needs, goals, preferences and schedule. Your Care Partner works with you and your family or carers to plan suitable support.
