01When to ask about support
Changes in memory, mobility, daily tasks or family care arrangements can be a reason to ask about aged care. You may also want support after a fall, hospital stay or new diagnosis. The appropriate service and eligibility are assessed individually.
021. Contact My Aged Care
Check your eligibility and apply through My Aged Care online or by calling 1800 200 422. JewishCare can help you navigate the process and discuss the support you need.
032. Take part in an assessment
If referred for an assessment, an assessor discusses your circumstances and care needs. The source describes in-person assessments at home. Confirm current arrangements with My Aged Care.
043. Understand your outcome
Your assessment outcome explains the government-funded services for which you are approved. Once a package is available, the JewishCare team gathers information about your needs and goals and develops a person-centred plan.
05Review and adapt
If your needs change, discuss this with your Care Partner. Your plan can be reviewed; a new assessment may be requested. Some people choose private services in addition to their funded support.
06Funding and costs
Support may be arranged through an approved Support at Home package or privately, depending on the service and your circumstances. Ask the team about your options and the current price guide. Assessment, contributions and service availability must be confirmed for your situation.
07Getting started
Contact JewishCare to discuss your needs. If you are enquiring on someone else’s behalf, make sure you have their permission. The current aged-care enquiry form accepts enquiries for yourself or another person and states a response within 2 business days.
