Personalised, multidisciplinary support for your health and independence at home.

JewishCare connects you with allied health professionals, including physiotherapists, nurses, speech therapists, dietitians and podiatrists. Support is shaped around your needs and care plan.
Physiotherapy can address mobility, functional strength, balance, confidence and pain management. Exercise physiology can support an individual rehabilitation or exercise plan. Ask the team which qualified professional is appropriate for your needs.
Physical and cognitive assessments identify needs and ways to support independence. Recommendations can include assistive equipment and home modifications for safety and comfort.
Speech therapists support communication and swallowing difficulties through treatment planning and ongoing care. Podiatrists assess and treat foot, nail, ankle and lower-leg concerns, including chronic and circulatory conditions.
The source describes support with mental health concerns and counselling related to palliative care, dementia, grief and financial difficulties. Speak to the team about the relevant service and referral pathway.
Care Partners listen to you and your family to develop a person-centred plan. Services can be reviewed as needs change, within available funding and programme rules. Our team supports cultural needs; many staff speak Russian, Hebrew, Polish or Hungarian as well as English.
Support may be arranged through an approved Support at Home package or privately, depending on the service and your circumstances. Ask the team about your options and the current price guide. Assessment, contributions and service availability must be confirmed for your situation.
Contact JewishCare to discuss your needs. If you are enquiring on someone else’s behalf, make sure you have their permission. The current aged-care enquiry form accepts enquiries for yourself or another person and states a response within 2 business days.
Talk with your Care Partner. Your care plan can be reviewed, services adjusted within programme rules and available funds, and a new assessment arranged where needed.
